On your first visit, you will need to complete a New Patient Form and Notice of Privacy Practice. Download and complete these forms ahead of time to help make the most out of your consultation time with Dr. Layton

consultation fee
Consulations are $65.00.

cancellation policy
We respect the value of your time and ask the same of you. Therefore, we request the courtesy of a 24 hour advanced notice for rescheduling or canceling your appointment. If this request is not honored, we reserve the right to ask for pre-payment of the consultation fee by credit card prior to rescheduling another appointment for you.

privacy
The privacy of your personal and medical information will be fully maintained by our office. Also, during the course of your surgical procedure and subsequent visits, we will arrange for your entry and exit through a private door to the office.
